1. The Players Club at Black Oak Casino membership enrollment is free to all Guests 21 years of age or older.
2. A valid photo I.D. is required for club enrollment and for any club transaction.
3. It is the player’s responsibility to properly insert their Players Club at Black Oak Casino card before play—Black Oak Casino will not be responsible for untracked play due to player neglect or improper insertion of the card.
4. Table Games players must present their Players Club at Black Oak Casino card before wagering.
5. Any and all discrepancies should be reported immediately to The Players Club at Black Oak Casino to be eligible for reconsideration. Black Oak Casino reserves the right to settle any discrepancy at its sole discretion.

9. Membership, along with any accumulated points or benefits is non-transferable.
10. Players Club at Black Oak Casino accounts with no activity for a period of 12 months will be considered abandoned and all unredeemed points will be cancelled.
11. By accepting membership, Guests are accepting all policies of The Players Club at Black Oak Casino.
12. Black Oak Casino reserves the right to modify or cancel this program at any time without prior notice
